Dear Patients,

Our 2024 carer’s review campaign has now begun, you will receive a self booking text message to book an appointment with a Nurse or HCA if you are eligible.

How to tell if you’re a carer

You’re a carer if you’re looking after someone regularly because they’re ill, they’re an older person or they’re disabled – including family members. You will initially need to tell us if you are a carer so that we can add you to our carer’s register. Please also inform us if you are no longer a carer so that we can remove you from the carer’s register. You will not be removed from the carer

Carers help with:

  1. Washing, dressing or taking medicines
  2. Getting out and about and travelling to doctors’ appointments
  3. Shopping, cleaning and laundry
  4. Paying bills and organising finances
  5. Watching over someone if they can’t be left alone

    Carer’s Emergency Cards

Carers emergency cards are free to all family/informal carer’s who provide care for someone who would be unable to look after themselves if their carer were suddenly unable to care for them, due to an accident, emergency or personal crisis. To apply visit: www.tinyurl.com/guvcic
